These replace the old Wix / 123 Form Builder pages. Paperless-ngx is the system of record. Submit is not wired yet; the fields document what we will POST.

Number porting requires a PDF upload (LOA / CSR / bill). That is a hard requirement, secondary only to getting ACH and LOA online first.
